Choose the type of problem first (e.g. Road / Pavement for a pothole, Lighting for a broken streetlight). Not sure? Choose “Other MEO-related Issue”.

Only facilities that fit the chosen category are listed. Choose “Other” if the affected facility is not listed.

Urgency is automatically determined by the LGU system based on the estimated number of affected residents — there is no manual urgency selection.

Separately, report submission is only available while your device is within the Municipality of Balatan; your device location is checked when you submit and is not saved with the report.
